Notice of Information Practices

April ABA (including April ABA LLC, April ABA of OH LLC, April ABA of Indiana LLC, and April ABA of Kentucky LLC) is committed to protecting the privacy and confidentiality of our clients' personal and health information. This Notice of Information Practices outlines how we collect, use, disclose, and safeguard your information in accordance with the Health Insurance Portability and Accountability Act (HIPAA).

  1. Collection of Information: We collect personal and health information about our clients that is necessary to provide Applied Behavior Analysis (ABA) services. This may include contact information, demographic information, health history, insurance information, and other relevant data.
  2. Use of Information: We use the collected information to provide, coordinate, and manage ABA services, to communicate with clients and their families, to bill for services, and to comply with legal and regulatory requirements.
  3. Disclosure of Information: We may disclose client information to third parties as necessary for treatment, payment, and healthcare operations. This may include sharing information with healthcare providers, insurance companies, and other entities involved in the client's care. We will obtain the client's authorization before disclosing information for purposes other than treatment, payment, or healthcare operations, unless required by law. We may also disclose client information without authorization for certain purposes permitted by HIPAA, such as for public health activities, legal proceedings, law enforcement purposes, and as required by law. Other uses and disclosures not described in this Notice will be made only with the client's written authorization, which may be revoked by the client in writing.
  4. Safeguarding Information: We maintain appropriate administrative, technical, and physical safeguards to protect the confidentiality, integrity, and availability of client information. Our staff is trained on privacy and security practices, and we regularly review and update our safeguards to ensure the protection of client information.
  5. Access to Information: Clients have the right to access, inspect, and obtain a copy of their health information maintained by April ABA. Requests for access should be made in writing to our Privacy Officer.
  6. Amendment of Information: Clients have the right to request an amendment to their health information if they believe it is incorrect or incomplete. Requests for amendment should be made in writing to our Privacy Officer.
  7. Accounting of Disclosures: Clients have the right to request an accounting of certain disclosures of their health information made by April ABA. Requests for an accounting of disclosures should be made in writing to our Privacy Officer.
  8. Restrictions on Uses and Disclosures: Clients have the right to request restrictions on certain uses and disclosures of their health information. However, April ABA is not required to agree to such restrictions. Requests for restrictions should be made in writing to our Privacy Officer.
  9. Confidential Communications: Clients have the right to request to receive confidential communications of their health information by alternative means or at alternative locations. Reasonable requests will be accommodated.
  10. Breach Notification: In the event of a breach of a client's unsecured protected health information, April ABA will provide notification to the affected client(s) in accordance with HIPAA requirements.
  11. Changes to this Notice: April ABA reserves the right to change the terms of this Notice and to make the new provisions effective for all protected health information maintained by the organization. We will provide clients with a revised Notice upon request and will make the revised Notice available on our website.
  12. Complaints: If a client believes their privacy rights have been violated, they may file a complaint with our Privacy Officer or with the U.S. Department of Health and Human Services Office for Civil Rights.

This Notice of Information Practices is effective as of 06/14/2024. We are required by law to maintain the privacy of protected health information, to provide this Notice of our legal duties and privacy practices, and to abide by the terms of the current Notice.
